If you are concerned about your weight, are suffering diabetes, or experiencing other chronic health conditions, you may be eligible for a Medicare rebate of $52.95 when seeing our Dietitian.
The Chronic Disease Management system (formerly known as Enhanced Primary Care or EPC) is a Medicare initiative which enables General Practitioners (GPs) to better coordinate care for patients experiencing a range of conditions such as diabetes, obesity, cardiovascular disease and many more.
By obtaining a referral from your regular treating GP, a CDM plan will enable you to receive a rebate of $52.95 from Medicare when you see an eligible allied health practitioner, including our Dietitian at The Low Carb Clinic (item 10954). This rebate is applicable for up to 5 visits each calendar year (i.e. 1 January to 31 December).
First, visit your regular treating GP to discuss your concerns. Ask them to establish a GP Management Plan (item 721) and a Team Care Arrangement (item 723) for you, as well as a CDM referral using this form.
Contact us to make an appointment, please advise our team that you hold a CDM referral so we can ensure the necessary paperwork is completed for your claim following your visit.
It is a Medicare requirement that the referral must be made by your regular treating GP, so regrettably our doctors at The Low Carb Clinic are unable to make the referral for you.
Where you are eligible for a rebate for Dietetic services under your private health extras cover, you may only claim a rebate through Medicare or your private health insurer. In most cases, the Medicare rebate will be higher, however it is worthwhile checking with your private health insurer.
As with other services claimed under Medicare, you must attend the clinic in person to be eligible to claim the rebate, therefore CDM rebates are not applicable to telephone or Skype appointments.
